What is Post-Hospital Care?


Care after a Hospital Discharge goes by many different names – including Post-Hospital Care, Transitional Care, hospital-to-home services, Post Surgery Care, and post acute care among others.

 

Please note:  Transitional care does not include medical needs such as wound management and rehabilitation. Medical needs can only be done legally by medically authorized personnel such as a nurse, regardless of the agency you go with. The goal of transitional care is to provide the necessary care and attention that is required outside of medical needs.

How Long Does It Last?


On average, transitional care can last anywhere between a few days to several months. The time range ultimately depends on the loved one’s condition and the recovery requirements.

    Post Discharge Care refers to the care and supervision that a client receives after being discharged from a hospital, or other medical facility such as a nursing home or rehabilitation center. This is separate from medical care, such as wound management and rehabilitation.

  • Who is eligible?

    Post Discharge Care is needed for anyone being released from a hospital or medical facility who requires further recovery outside of the facility. Examples include clients recovering from surgery, an illness, an injury, or a newly diagnosed condition that requires ongoing maintenance and lifestyle changes.

  • What’s the difference between this and home care?

    Transitional care refers to the process of transferring care responsibilities of a client from one setting to another, such as from a hospital to their family. Home care refers to the setting of where care is being received – in this case, a client’s personal home. Home care is very common in the process of transitional care. 

  • What services are included?

    Our transitional care services include assistance with activities of daily living such as meal preparation, medication reminders, grooming, bathroom assistance, and transportation to and from appointments. Our caregivers can also assist with light housekeeping, shopping, laundry, and more.

  • What’s the difference between this and post surgery care or hospital-to-home?
